Hamilton Health Center is seeking a Housing Case Manager to support individuals experiencing opioid use disorder by promoting housing stability, recovery, and long-term well-being.
In this role, you will provide case management and care coordination services, helping clients navigate housing resources, connect to community supports, and achieve individualized recovery goals. Provide case management, care coordination, and advocacy to support client recovery and housing stability
Develop and implement individualized service and housing plans with clients
Connect clients to housing resources, employment support, counseling, and community services
Maintain relationships with landlords and community partners to secure housing opportunities
Monitor client progress and adjust service plans as needed
Document all services and interactions accurately and in a timely manner
Support clients in overcoming barriers to care, including transportation needs
Collaborate with healthcare providers, agencies, and community organizations
Participate in required meetings, trainings, and outreach activities
Ensure compliance with confidentiality, program guidelines, and regulatory requirements
Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
Minimum of 10 years of relevant experience
Willingness to enroll in a related Bachelor’s degree program within 6 months of hire
At least 1 year of case management experience preferred
Experience working with substance use disorder, housing services, or vulnerable populations is a plus
Community-based role with travel throughout the service area
Direct client interaction in various settings (clinic, community, housing sites)
May involve supporting clients with complex social and behavioral health needs
