Position: Housing Case Manager
Mission
THAP works to provide safe housing and appropriate supportive services to transgender and gender nonconforming (defined as any personal gender identity different or transgressive from the sex assigned at birth) individuals who are homeless to facilitate movement to independent living and promote productive employment and reduction of risky behaviors, or who seek intermediate or long-term housing that’s centered on the needs of transgender and gender nonconforming people.
General Description
The THAP housing case manager will assist clients in obtaining and maintaining the best possible housing solution for their individual circumstances through case management, advocacy, and referrals. In addition to this, the case manager will ensure that clients receive sufficient information and access to community resources by creating and maintaining an efficient case management program.
Responsibilities
- Represent THAP in a positive manner with clients, suppliers, members, volunteers, and the communities served. Interact effectively with a diverse group of faculty, staff, and clients
- Handle all information with tact and discretion and recognize the confidential nature of THAP.
- Participate in staff meetings, in-service training, and workshops deemed appropriate by the Chief Financial Officer.
- Screen all applicants and determine eligibility at intake and when indicated by program.
- Make appropriate referrals as needed.
- Orient all clients and guests on THAP program policies and procedures.
- Create case plans and provide Case management for individual guests and clients including: conduct needs assessments, create individualized service plans to achieve self-sufficiency.
- Serve as the program representative at team meetings presenting the issues and concerns of the guests and clients.
- Increase client’s awareness of available community and agency resources and assist them in accessing these services.
- Develop and maintain partnerships and networks with community agencies that best serve the general specific needs of THAP clients.
- Maintain all client charts and records according to established standards.
- Complete data collection and reporting requirements for various funding sources utilizing the required data systems.
- Keep detailed financial records of guest and client-related transactions including date, demographic information, amounts spent, and receipts and invoices up to date.
- Submit weekly reports to Program Manager and/or Chief Financial Officer.
- Help clients maintain current housing, including utility bill assistance, eviction notices, etc.
- May assist with grant writing and other reporting activities.
- Manage program budgets for security deposits, rent, service match, etc.
- Assist the leadership with table talk, fundraisers, and other public-facing functions as well as assist with management of social media accounts.
- Perform other duties as assigned.
Job Requirements
- Demonstrated experience working with trans and gender non-conforming individuals, people of color, and immigrants.
- BA in social science, human services, and/or social work – preferred, not required.
- 1-3 years of experience in nonprofit administration, human services, or related fields.
- Access to reliable transportation.
- Familiarity with housing first principles, data analysis, and program evaluation.
- Must work well independently as the position has limited supervision.
- Proficiency in Microsoft applications including Word, Excel, Google Workspace, and HMIS reporting systems.
- Strong organizational, verbal and written communication skills.
- Ability to establish and maintain relationships with community partners.
- Excellent customer service skills and attention to detail.
- Ability to work in high intensity and emotional atmospheres where clients seek assistance. Conflict resolution and verbal de-escalation training preferred.
- Familiarity with social service and state agencies providing assistance programs.
- Competent and ethical management of organization funds and spending accounts.
