PURPOSE:
Seeking to make a positive difference in the life of youth ages 18-25? Vita Nova is seeking a passionate, energetic Housing Case Manager to join our dynamic team! Our Youth Homelessness Demonstration Program (YHDP) Transitional Housing (TH) to Rapid Re-Housing (RRH) program provides individualized case management services, innovative programming, and principles of positive youth development to assist former Foster Care and other disconnected homeless youth in achieving stable and safe housing. The project focuses on four core outcome areas: establishing stable housing, education/employment attainment, creating connections and an increasing social and emotional well-being.

KEY RESULT AREAS:

Partner with youth to create an individualized service goal plan to include meeting basic needs, service coordination, life skills development, social & emotional wellbeing, permanent connections and plan for safe and stable housing.

Facilitate weekly individual and group Life Skills training in the home and community.

Administer TAY-VI-SPDAT vulnerability index and service prioritization to homeless youth.

Maintain documentation and reporting standards: track census, program eligibility, participation and challenges in the homeless management information system (HMIS).

Educate participants on landlord/tenant rights and responsibilities, lease agreements, pest control and apartment maintenance.
- Assistance in maintaining apartments in clean, well-maintained condition.

Recruit, train and orient volunteers and mentors serving as permanent connections.

Connect youth to needed services including but not limited to, mental health, physical/dental health services, substance abuse, domestic violence and social supports.

Coordinate case staffings and team meetings to assess progress and develop ongoing support teams.
- Offer ongoing skill building for employability, education and career planning.

Provide transportation and community based services in partnership with the youth participant.

Attend community collaboration and networking meetings to expand services offered to youth.

Qualifications:
- Bachelor's Degree in Social Work or human services related field.

Understand and implement trauma informed care practices and positive youth development framework.

Ability to work non-traditional hours, including evenings and weekends to meet youth needs.

Participate in on-call rotation to address roommate conflict or resolve crisis in the home.

Resourceful in securing community resources for basic needs and community services.

Self-directed and able to create Life Skills curriculum, lesson plans, and must be comfortable presenting to small and large groups.

Possess critical thinking skills and the ability to assess crisis and create safety plans.
