Position Summary:
The Housing Case Manager is responsible for delivering comprehensive, client-centered services to individuals and families experiencing homelessness or housing instability. This role emphasizes housing stabilization, resource coordination, and sustained support to promote long-term self-sufficiency. The position requires adherence to established best practices in case management, as well as the maintenance of accurate, timely, and compliant documentation within the Homeless Management Information System (HMIS).


Key Responsibilities:

  • Conduct comprehensive client assessments to identify housing needs, barriers, strengths, and appropriate interventions.
  • Develop and implement individualized housing service plans in collaboration with clients.
  • Assist clients in identifying, securing, and maintaining safe, stable, and affordable housing.
  • Coordinate referrals to community resources, including employment services, healthcare providers, and social support agencies.
  • Monitor client progress and modify service plans as necessary to ensure positive and sustainable outcomes.
  • Maintain consistent and professional communication with landlords, service providers, and other relevant stakeholders.
  • Ensure accurate, timely, and compliant data entry, case notes, and documentation within HMIS, in accordance with federal, state, and organizational standards.
  • Participate in case reviews, interdisciplinary team meetings, and ongoing professional development activities.

Minimum Qualifications:

  • A minimum of two year of experience in case management, housing services, or a related field.
  • Demonstrated knowledge of Housing First principles and trauma-informed care practices.
  • Strong organizational, communication, and interpersonal skills.
  • Ability to manage a diverse caseload while consistently meeting documentation and reporting deadlines.
  • Experience working with vulnerable populations, including individuals experiencing chronic homelessness.

HMIS Knowledge and Requirements:

  • Demonstrated understanding of HMIS policies, data standards, and confidentiality requirements.
  • Experience entering, updating, and maintaining accurate client-level data within HMIS.
  • Ability to generate reports and utilize HMIS data to inform service delivery and program outcomes.
  • Commitment to maintaining high standards of data quality, including completeness, accuracy, and timeliness.
  • Knowledge of applicable privacy regulations, including informed consent procedures and data security protocols.

Preferred Qualifications:

  • Bachelor’s degree in Social Work, Human Services, or a closely related field, or equivalent professional experience.
  • HMIS certification or demonstrated hands-on experience with HMIS platforms.

Physical Requirements:

  • Ability to lift up to 20 pounds, as required.
  • Participation in community outreach activities that may involve prolonged periods of standing.
  • Access to reliable transportation for travel to partner agencies and client-related activities.
  • Ability to maintain a flexible schedule based on departmental needs.
  • Capacity to work effectively under varying environmental and weather conditions.
