IKRON Greater Seattle is looking for a housing case manager to join our multi-disciplinary PACT Team serving the East side of King County! The PACT Team is a clinical team that provides psychiatric wrap-a-around treatment for clients with many barriers. The case manager will provide case management services to a caseload clients with mental health diagnoses.
POSITION SUMMARY:
Bring housing case management knowledge to the PACT team and support PACT clients with finding and maintaining housing. Provide individual supportive services in an integrated multidisciplinary team environment under the clinical supervision of licensed staff members. Contribute to comprehensive assessments for individuals with severe mental illness and/or substance abuse problems. Assist in developing and reviewing Individualized Service Plans. Provide a range of case management services related to housing to a caseload of clients. These case management services can include, scheduling appointments, assisting clients with application paperwork, outreach, accessing resources in the community, housing assistance, and collaborating with community partners and medical providers. Document all progress notes in a timely manner. Consult with staff regarding treatment and prognosis of individuals served. Stay in good communication with the team regarding client’s progress. Attend clinical and administrative staff meetings. Other duties upon request.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Be a member of the individual treatment team for an assigned number of clients.
Provide housing case manage services for an assigned number of clients.
Participant in the client-centered comprehensive assessment and individual treatment plans.
Consult with community agencies and families to maintain coordination in the treatment process.
Document client progress to maintain a record of client activity according to established methods and procedures.
Participate in providing treatment and dual diagnosis services.
QUALIFICATIONS / REQUIREMENTS:
1. Bachelor’s Degree in Counseling, Social Work, or related discipline preferred.
2. High school degree required.
3. A minimum of one year of experience working with adults having moderate to severe mental illness.
4. Knowledge of housing resources in King County, particularly the Eastside of King County

Pay: $23.00 - $26.00 per hour
