Ascencia is a nonprofit homeless services agency based in Glendale, California, dedicated to helping individuals and families across L.A. County move from homelessness to permanent housing. Through street outreach, permanent supportive housing, case management, mental health care, and other supportive services, we provide compassionate, comprehensive support. This temporary role is expected to continue through August 31, 2026, at which time the assignment will end. The Housing Case Manager role provides case management and supportive services for individuals experiencing homelessness with disabilities, including serious mental illness, physical disabilities, chronic substance use, and AIDS or related diseases. This position supports the City of Glendale’s Shelter Plus Care program, helping clients access and maintain permanent housing. The Housing Case Manager uses harm reduction strategies to enable participants to retain their rental housing and works collaboratively with staff, case managers, and community resources to ensure client-centered, comprehensive care.

Salary: $23.10 per hour
Provide ongoing support and individualized case management for clients experiencing homelessness.
Develop, implement, and continually evaluate individualized housing plans, adjusting plans as needed.
Assist clients in accessing resources and services to achieve case management goals, including healthcare, legal advocacy, and benefits such as SSI/SSDI.
Conduct crisis intervention and provide guidance in urgent situations.
Adhere to professional ethics and maintain strict boundaries in client interactions.
Prepare case management reports, including outcomes and program successes.
Develop housing listings and coordinate client moves to permanent housing.
Survey the rental market for affordable housing and advocate for clients with prospective landlords.
Maintain accurate documentation of service objectives, client outcomes, and program activities in compliance with federal, state, county, and agency guidelines.
Maintain client data systems, including HMIS entries and case notes.
Participate in staff, program-related, community, coalition, and committee meetings as assigned.
Bachelor’s degree in human services, social work, psychology, or equivalent combination of education and experience required.
Experience in social service or homeless services settings with case management systems and planning techniques.
Knowledge of housing resources, tenant rights, and the Continuum of Care for homeless persons.
Experience working with homeless individuals and families preferred.
Proficiency in Microsoft Word and Excel; ability to learn HMIS.
Must have reliable transportation, valid driver’s license, and meet agency insurance requirements.
Drug and TB testing required prior to employment.
Travel to field locations to carry out job responsibilities.
Prolonged sitting, standing, bending, stooping, stretching, and occasional lifting (up to 25 pounds).
Frequent use of computer, keyboard, and phone for documentation and communication.
