Housing Navigator/Case Manager

Position Summary

The Housing Navigator/Case Manager of DayBreak holds a key role in a small yet very dynamic organization with a critical mission. The Housing Navigator/Case Manager is someone who knows health and social services in the community and works closely with clients to help them regain or maintain their housing, health, and well-being. The Housing Navigator/Case Manager will connect with the referred senior within 48 hours, conduct an initial assessment, and walk the elder through every step of getting the resources they need, taking into consideration needs, wishes, existing support systems, and other factors that can determine better health outcomes.

As a member of a small team, the Housing Navigator/Case Manager will also take part in other activities we will have, such as events and other tasks in the office as needed.

Key Responsibilities

  • Work closely with APS, hospitals, and EMS teams to provide timely case management to older adults.
  • Work with the Housing and Case Management team to identify, develop and assess the tools and resources needed to serve elder clients
  • Coordinate the care plan with multiple partners and provide ongoing coordination of support services for clients and their families.
  • Perform client assessments, develop treatment plans, and participate in meetings and case reviews as required.
  • Arrange all services as appropriate, monitor their effectiveness, and reassess client needs, including follow-up on the care plan execution.
  • Intervene in acute situations when community services are not immediately available, including the provision of transportation, emergency food, or other appropriate services.
  • Maintain program documentation and ensure the completion of all required reports in a timely manner.
  • Perform other duties as assigned.

Qualifications

  • A bachelor's degree or MSW desired, but not required
  • Experience in social welfare, gerontology, human development, or a related discipline.
  • Familiarity with local hospitals' work environment and ability to work closely with medical professionals
  • Excellent background and knowledge of issues related to Geriatrics desired
  • Ability to interact effectively with clients, caregivers and professionals of all backgrounds
  • Excellent communication skills with a compassionate approach to service delivery
  • Motivational interviewing experience desired
  • Working knowledge of local resources, and experience working with social service agencies and senior services desirable
  • Ability to work in a highly collaborative environment, and be part of a small office team
  • Ability to work independently, think ‘out of the box,' and be self-motivated.
  • Ability to prioritize and manage multiple tasks, and to perform under conditions requiring speed and accuracy
  • Excellent speaking, writing, and organization skills
  • Computer literate and ability to use multiple online data systems (i.e., Slack, Google Drive, online client databases)
  • Valid California driver's license, automobile insurance and access to an automobile
  • Ability to pass a background check

Physical requirements:

  • Sitting: 45 to 1 hour while driving within the community
  • Intermittent stooping 5 to 10 minutes
  • Intermittent standing within the 8-hour shift
  • Intermittent reaching within an 8-hour period
  • Intermittent typing within an 8-hour period
  • This job does involve some repetitive motion within an 8-hour shift
  • This job does involve walking while in the community
  • This job does involve intermittent sitting while in the community with clients.
  • This job does involve some lifting up to 25lb items such as walkers, boxes, wheelchairs, scooters, other items as identified

Location: DayBreak's office is located in DT San Leandro. However, this position requires traveling to different locations in Alameda County.

Hours and Compensation: This is a full-time, 100% FTE position. The starting salary is $65,000-$70,000 and is competitive with small-sized nonprofits.

Benefits: Benefits include medical and dental insurance, eleven paid holidays, and accrued sick & wellness leave and vacation time.
