Human Services Case Manager (QIDP)

Position Overview
We are seeking a Human Services Case Manager (Qualified Intellectual Disabilities Professional – QIDP) to join our residential team. This role oversees and coordinates care for residents in our ICF-DD residential facility, ensuring services meet each individual’s needs, goals, and regulatory requirements.

The Case Manager works closely with residents, direct support staff, families, and outside agencies to ensure safe, person-centered, and compliant residential programming.

Key Responsibilities

• Coordinate and monitor residential services for individuals in our ICF-DD facility
• Develop, implement, and monitor Individual Service Plans (ISPs)
• Conduct assessments and track progress toward individual goals
• Ensure compliance with all state and federal regulations for ICF-DD programs
• Maintain required documentation and prepare reports for licensing and audits
• Communicate regularly with families, guardians, and interdisciplinary team members
• Advocate for residents to promote independence, community involvement, and quality of life

Qualifications

• Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, Special Education, or a related field
• Ability to qualify as a QIDP under Illinois standards
• Experience working with individuals with developmental or intellectual disabilities preferred (minimum 1 year)
• Strong documentation, organization, and communication skills

Pay: From $22.00 per hour
