Carries a case load of youth that are on Youth Justice Supervision through the Juvenile Court System. This position requires that the employee assess the individual and/or family risk and need factors through the use of the Youth Assessment Screening Instrument (YASI). The employee will use a variety of social work practices including Functional Family Case Management, which is a process designed to motivate families to change and link them to services. Will also utilize community based interventions in order to balance the rehabilitative needs of the child and family with public safety concerns.

Act as a case manager for youth and their families by providing community based client contact and supervision including regular home visits based on the

YASI risk level and based on Functional Family Case Management (FFCM).

Participate in the FFCM case consultations on a weekly basis.

Prepare written reports and other oral reports to the court when necessary.

Complete a YASI assessment and case plan for each youth.
-
Conduct YASI reassessments and reviews of the case plan every 90 days.

Meet monthly with employee's supervisor for case consultation which includes discussing FFCM process.

Maintain case records in eWisacwis and in the youth's file that are in compliance with the Juvenile Justice Code.

Consult and interact with schools and other community based agencies including other Rock County Human Service Units.

Refer families to necessary programs and resources as deemed appropriate through the youth's case plan.

Attend and participate in all staffings related to the youth.

Provide Transportation to the youth and family when necessary.

Maintain regular collaboration with the Intensive Case Manager (ICM) and the Electronic Monitoring Program (EMP) Coordinator (if involved).
10% - Collaboration with EMP Coordinator, ICM, and Diversion Programs-
Will have contact with the EMP coordinator and ICM worker if youth is involved in either program.

Will have regular contact with Diversion workers if youth is involved in Diversion Programming.

5% - Intake duties/Court Responsibilities-
Provide day-time intake custody decisions and weekly court coverage when assigned.
