Hybrid Registered Nurse

Job Summary
We are seeking a dynamic and compassionate Hybrid Registered Nurse to join our innovative healthcare team. Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories using advanced EMR (Electronic Medical Record) systems such as Epic and Cerner.
• Administer medications via injections, IV infusions, and other routes while ensuring aseptic technique and adherence to HIPAA privacy standards.
• Perform specialized procedures including suturing, catheterization (urinary and vascular), tube feeding management, and ostomy care.
• Manage acute pain and infectious disease cases with appropriate interventions, triage protocols, and documentation aligned with NCQA standards.
• Coordinate care plans through case management practices for diverse populations such as geriatrics, pediatrics, behavioral health patients, and individuals with developmental disabilities or memory care needs.
• Support hospital-based activities including ICU, trauma medicine at Level I & II trauma centers, emergency room procedures, and post-acute care transitions.
• Collaborate seamlessly with multidisciplinary teams across outpatient clinics, inpatient units, home care services, assisted living facilities, and long-term care environments.
• Utilize EMR/EHR systems like Athenahealth or eClinicalWorks for accurate documentation of patient care activities and medical records management.
• Engage in health coaching sessions focusing on chronic disease management such as diabetes or hypertension while educating patients on medication administration and lifestyle modifications.
• Assist in diagnostic evaluations involving laboratory specimen collection/processing and basic laboratory experience to support clinical decision-making.

Skills

• Extensive clinical experience with Epic, Cerner, or similar EMR/EHR systems; proficiency in ICD-9/10 coding for accurate documentation.
• Strong background in various specialties including critical care (ICU experience), emergency medicine (ER experience), cardiology (cardiac catheterization), nephrology (dialysis), geriatrics (memory & dementia care), pediatrics (neonatal ventilator), behavioral health, hospice & palliative medicine.
• Skilled in performing complex procedures such as suturing, venipuncture (phlebotomy), IV insertion/infusion therapy, sterile processing techniques, ventilator management, and spinal taps.
• Knowledge of infection control protocols and aseptic techniques essential for safe patient care across multiple settings.
• Familiarity with medical terminology related to trauma medicine, occupational health, home & community care experience; understanding of CPT coding for billing purposes.
• Ability to assess patients’ physiology accurately through vital signs monitoring and physical examinations; adept at working with individuals with disabilities or developmental challenges.
• Excellent communication skills for effective patient education on medication administration, pain management strategies, discharge planning, and health coaching.
• Capable of working independently or collaboratively within fast-paced environments such as hospitals (including Level I Trauma Centers), outpatient clinics, urgent care centers, or long-term care facilities like nursing homes or assisted living communities.
