The
Intensive Case Management Services (ICMS) Case Manager
will provide field-based, trauma-informed, and harm reduction-centered support to individuals referred through DHS and the Pathway Home team. The ICMS Case Manager will serve as a critical member of the Pathway Home team, working with individuals experiencing homelessness and frequent law enforcement contact. The role focuses on providing person-centered support to meet participants' individual needs, reduce criminal justice involvement, and support housing retention and health stability.
Schedule:
Monday-Friday 8am-4:30pm (40hours/week)
Pay Rate:
$27-$28 per hour
EDUCATION/EXPERIENCE/TRAINING
Required:

A bachelor's degree in social work, human services, public administration, or a related field

One years of experience in social services or healthcare, including experience dealing with substance

Valid CA Driver's License, auto insurance, and reliable transportation (Driving is an essential function of this position.) Must be able to meet and sustain eligibility for insurance coverage through First Day's automobile insurance policy;
Preferred:

Two years of non-profit experience with the homeless population and/or case management.

CPR/First Aid Training Certification and HMIS Training Certification (training provided).
Some of the responsibilities include:
SUPPORTIVE SERVICES

Conduct housing navigation services including assistance with applications, viewings, lease-ups, and move-in coordination.

Assist with gathering documents such as ID, Social Security Card, proof of income, and other housing eligibility documentation.

Provide harm reduction supplies, overdose prevention training, and linkages to MAT services as appropriate.

Maintain minimum of two (2) documented services per month per participant, including at least one (1) in-person visit.

Offer crisis intervention, transportation assistance, and facilitate life skills development.

Ensure all services and contacts are documented in the County-approved information system within three (3) business days.

Provide field-based ICMS-HR services including outreach, engagement, enrollment, assessment, care planning, service delivery, and case closure aligned with LEAD SOW.

Receive referrals from LEAD referral pathways and respond within one (1) business day.

Attempt at least three (3) in-person contacts with referred individuals within five (5) business days .

Conduct informed consent processes and obtain written consents for service and care coordination.

Complete Initial Assessments and Individualized Care Plans within 30 days of enrollment and update quarterly.
OUTREACH AND RELATIONSHIP MANAGEMENT

Collaborate with Community Engagement Specialists to establish strong relationships with local businesses, churches, social service organizations, service clubs, and the Police Department. This collaboration aims to help these entities understand and aid homeless and at-risk individuals eligible within the Los Angeles Continuum of Care (LA CoC) and Housing Authority of the County of Los Angeles (HACoLA) communities, including La Mirada, Montebello, Santa Fe Springs, Pico Rivera, Whittier, and surrounding County unincorporated areas.

Respond to outreach requests from local community partners to assist clients experiencing homelessness.

Attend collaborative meetings.

Regularly meet with supervisor to discuss case management plans and professional goals.

Actively participate in internal and external staff meetings and trainings.

Engage with collaborating agencies and disciplines that work with clients experiencing homelessness to connect them with suitable services.
