IDD Case Manager

The Case Manager will be responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with cognitive disabilities, assembling information about the individual for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the planning, coordination, and monitoring of services and supports provided to individuals with cognitive disabilities and their families as appropriate. Spanish bilingual preferred with a $1.00 increase in pay rate.

Requirements:

• Bachelor’s degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabilities.
• Driving is a requirement for this position.
• Prior case management and/or advocacy experience strongly preferred.
