Job Summary:

As an In-Home Outreach Counselor/Case Manager, you'll be the heart and soul of our Family Preservation team, delivering essential in-home case management services to our clients. Your role will be pivotal in ensuring child safety, preventing abuse and neglect, and helping families thrive through personalized treatment plans.
What You'll Do:
- Be the Guiding Light: Provide weekly in-home outreach counseling, initial assessments, and case management services to assigned clients.
- Welcome Wagon: Orient clients to our program philosophy, expectations, services, and treatment process. You'll be their go-to for rules, regulations, and client rights.
- Safety First: Observe family environments to ensure the safety of children in their homes.
- Tailor-Made Plans: Develop individualized service plans based on assessment reports.
- Family Time: Conduct individual and family sessions as needed to meet family needs.
- Progress Reports: Evaluate parent/child/family progress and provide detailed progress letters to LA County Social Worker (CSW).
- Communication Hub: Keep the LA County Social Worker (CSW), Program Manager, and/or Clinical Supervisor informed about observed behavior, progress, and any issues.
- Resource Connector: Assist families in identifying and accessing community resources.
- Transition Wizard: Arrange smooth transitional planning for families upon service completion.
- Team Player: Participate in Multidisciplinary Case Planning Committee (MCPC) meetings.
- Documentation Guru: Document weekly visits and submit reports to quality assurance staff.
- Ongoing Learner: Attend weekly clinical supervision, staff meetings, and professional development trainings.
- Flexible Helper: Assist with other duties as needed.
What You Bring:
- Educational Background: Bachelor’s or Master’s Degree in Social Work, Child Counseling, Development, Psychology, or Human Services preferred.
- Experience: At least two years of case management experience in a social services agency or mental health setting preferred.
- Communication Skills: Ability to interact and communicate effectively with emotionally disturbed adolescents, parents, foster parents, schools, other staff, physicians, and public agencies.
- Documentation Skills: Proficiency in clear, concise, and timely documentation and correspondence.
- Tech Savvy: Proficiency in data entry computer databases.
- Interpersonal Skills: Excellent written, verbal communication, and interpersonal skills.
- Multi-Tasking Ability: Ability to multi-task, meet deadlines, and work with all levels of management.
- Reliable Transportation: Must have reliable transportation and provide valid proof of current insurance coverage according to Project Impact program standards.
