Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health by observing vital signs, medication adherence, and overall well-being, while maintaining detailed care plans
• Provide companionship and emotional support to promote mental health and social engagement
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
• Support clients with mobility using equipment such as Hoyer lifts or other assistive devices, ensuring safe transfer techniques
• Maintain a clean and safe environment by performing light housekeeping tasks including cleaning, laundry, and organizing personal spaces
• Document patient observations accurately in compliance with HIPAA regulations and report any concerns to supervising healthcare professionals

Qualifications

• Prior experience in home & community care or assisted living settings is highly preferred
• Knowledge of patient observation techniques, medication administration, and vital signs monitoring
• Certification in First Aid and CPR is required; additional certifications such as Alzheimer's or dementia care are a plus
• Experience working with individuals with developmental disabilities, autism, or memory care needs is advantageous
• Ability to lift heavy objects safely and perform physically demanding tasks such as transfers using Hoyer lifts or similar equipment
• Strong communication skills to effectively collaborate with clients’ families and healthcare teams
• Valid driver’s license and reliable transportation are often necessary for client visits
