Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using transfer belts.
• Monitor patients’ vital signs and observe changes in health status, reporting any concerns promptly to healthcare professionals.
• Support clients with memory care needs, including dementia and Alzheimer’s care, using specialized techniques to promote comfort and safety.
• Maintain detailed documentation of care provided, patient observations, and updates in compliance with healthcare regulations.
• Perform and maintaining a safe environment to promote overall well-being.

Qualifications

• Prior experience working in home & community care settings or assisted living facilities is highly preferred.
• Experience working with individuals with developmental disabilities, autism, dementia, or hospice needs is advantageous.
• Ability to lift heavy objects safely.
• Strong communication skills with the ability to follow detailed care plans and document observations accurately.
