Duties

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
• Monitor patients’ health by observing vital signs, recognizing changes in condition, and reporting findings accurately
• Administer medications following care plans and HIPAA regulations to ensure patient safety and compliance
• Prepare nutritious meals tailored to dietary needs while maintaining a clean and safe kitchen environment
• Utilize specialized equipment such as Hoyer lifts or resident’s <PERSON> to safely transfer clients with mobility challenges
• Support clients with memory care needs related to Alzheimer’s disease or dementia through engagement and supervision
• Maintain detailed documentation of care provided, patient observations, and adherence to individualized care plans

Requirements

• Proven experience in home & community care, assisted living, nursing home settings or group homes is highly preferred
• Knowledge of patient monitoring techniques, medication administration, and vital signs measurement
• Certifications such as CPR, First Aid, HIPAA compliance training are required or highly desirable
• Experience working with individuals with developmental disabilities, autism spectrum disorder or dementia care is a strong plus
• Ability to perform heavy lifting and physical tasks safely while maintaining patient comfort and safety
• Strong communication skills for effective interaction with clients, families, and healthcare professionals
• Valid driver’s license and reliable transportation may be necessary depending on client location

Pay: $1,500.00 - $2,000.00 per month
