Job Summary

Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
  • Monitor patients’ health status by checking vital signs and observing for any changes or concerns
  • Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality
  • Prepare nutritious meals tailored to individual dietary needs and preferences, supporting overall health and wellness
  • Operate mobility equipment such as Hoyer lifts and assist with transfers to promote safety during movement
  • Maintain detailed documentation of care provided, patient observations, and any incidents in compliance with care plans and regulations
  • Support clients with cognitive impairments such as dementia or Alzheimer’s by implementing specialized memory care techniques
  • Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living environment
  • Collaborate with healthcare professionals to develop and follow personalized care plans that meet each client’s unique needs
  • Provide emotional support and companionship to foster social engagement and reduce feelings of isolation

Skills

  • Proven experience in home & community care, assisted living, nursing home settings, or group homes
  • Knowledge of patient observation techniques, medication administration, and vital signs monitoring
  • Strong understanding of Alzheimer’s care, dementia care, hospice care, and working with individuals with developmental disabilities or autism
  • Certified in CPR and First Aid; familiarity with HIPAA compliance is essential
  • Ability to perform heavy lifting safely when required for patient transfers or mobility assistance
  • Excellent communication skills to effectively interact with clients, families, and healthcare teams
  • Demonstrated ability to follow detailed care plans while maintaining flexibility for changing needs
  • Compassionate attitude combined with energetic motivation to provide high-quality caregiving services

Pay: $24,000.00 - $29,000.00 per year
