Responsibilities

• Deliver personalized care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
• Monitor patients’ health by observing vital signs, managing medication administration, and reporting any changes to healthcare professionals
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and preparation
• Maintain a clean and safe environment through regular cleaning, laundry, and household organization
• Support patients with mobility using equipment such as Hoyer lifts or other assistive devices while ensuring safety protocols are followed
• Document care activities accurately in compliance with HIPAA regulations and care plans
• Provide emotional support and companionship to promote mental well-being and social engagement

Qualifications

• Prior experience in home health, assisted living, nursing home settings, or group homes is highly preferred
• Knowledge of patient observation techniques, dementia care (including Alzheimer’s and memory care), hospice care, and working with individuals with developmental disabilities or autism
• Certification or training in CPR, First Aid, HIPAA compliance, and medication administration is advantageous
• Ability to perform heavy lifting safely when needed for patient transfers or mobility assistance
• Strong communication skills to effectively collaborate with healthcare teams, patients, and families
• Demonstrated compassion, patience, and respect for diverse populations with varying needs
• Valid driver’s license and reliable transportation are often required for travel between client homes
