Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by taking vital signs and observing changes in condition; report findings accurately
- Administer medications following care plans and HIPAA regulations to ensure safety and compliance
- Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking
- Support clients with mobility using equipment such as Hoyer lifts or wheelchairs; perform heavy lifting safely when necessary
- Maintain cleanliness of the living environment through light cleaning duties to promote hygiene and comfort
- Document care activities, update care plans regularly, and communicate effectively with healthcare teams or family members
Skills
- Proven experience in home & community care, assisted living, nursing homes, or group homes setting
- Strong knowledge of patient observation, medication administration, and vital signs monitoring
- Familiarity with specialized care such as Alzheimer’s, dementia, memory care, autism spectrum disorder, or developmental disabilities
- Certified in CPR and First Aid; experience with emergency response procedures
- Ability to work independently while following detailed care plans; excellent communication skills for reporting changes
- Physical stamina for heavy lifting and assisting with mobility; comfortable working in various home environments
- Knowledge of HIPAA regulations to ensure client confidentiality at all times
