In-home Medical Provider

Job Overview
Join our dynamic healthcare team as an In-home Medical Provider, where you will deliver comprehensive medical care directly to patients in their homes. We are seeking Nurse Pracitioners or Physican Assistants at this time.

This role offers a unique opportunity to combine clinical expertise with compassionate patient engagement, ensuring high-quality care for diverse populations including seniors, individuals with disabilities, and those requiring post-acute or chronic disease management. You will be instrumental in conducting assessments, administering treatments, and coordinating care plans—all within the comfort of patients’ homes. Your energetic approach and clinical proficiency will help improve health outcomes and enhance patient satisfaction.

Responsibilities

  • Perform thorough patient assessments, including vital signs, physical examinations, and detailed medical histories.
  • Assist patients with medication refills, medication management (we do not prescribe controlled substances) .
  • Manage and coordinate referrals for complex care needs including wound care, catheterization, tube feeding, ventilator management, and spinal taps.
  • Utilize EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Epic, eClinicalWorks, and Cerner to document patient encounters accurately and efficiently.
  • Coordinate with multidisciplinary teams on chronic care management, medication administration, and utilization review to optimize patient outcomes.
  • Conduct in-home consultations and triage to assess urgent needs or escalate cases to appropriate facilities or specialists.
  • Ensure compliance with HIPAA regulations and infection control protocols while maintaining patient confidentiality at all times.

Experience

  • Proven clinical experience in acute care settings such as ICU, emergency room, trauma centers (Level I & II), or hospital medicine.
  • Hands-on experience working with diverse populations including geriatrics.
  • Familiarity with medical procedures such as phlebotomy, venipuncture, sterile processing, laboratory specimen collection/processing, and diagnostic imaging.
  • Strong background in managing complex cases involving infectious disease care, pain management (including acute pain), occupational health issues, and behavioral health conditions.
  • Experience working within telehealth platforms and EMR/EHR systems like Athenahealth or eClinicalWorks is highly desirable.
  • Demonstrated ability to perform advanced procedures such as gastroscopy (gastric lavage), cardiac catheterization support, or ophthalmology-related treatments is a plus. Join us in making a difference by providing expert medical care directly where patients need it most—at home!

  • Hourly rate that includes travel time to and from scheduled home visits
