Responsibilities
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support
- Monitor patient health by checking vital signs and observing for changes in condition
- Administer medication and follow care plans accurately and safely
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Maintain cleanliness of clients’ living spaces through light housekeeping and sanitation tasks
- Support clients with memory care needs, including dementia or Alzheimer’s care, ensuring safety and comfort
- Use equipment such as Hoyer lifts or other assistive devices to safely transfer clients
- Document care activities meticulously in compliance with HIPAA regulations and agency policies
- Provide emotional support, companionship, and social engagement to promote mental well-being
- Follow all safety protocols including CPR and first aid procedures when required
Experience
- Prior experience in home health, hospice care, assisted living, or nursing home environments preferred
- Experience working with individuals with developmental disabilities, autism, or dementia strongly valued
- Knowledge of patient monitoring techniques, medication administration, and vital signs assessment
- Familiarity with care plans development and implementation for long-term or short-term care needs
- Demonstrated ability to handle heavy lifting safely and operate medical or mobility equipment effectively
- Previous work involving HIPAA compliance ensuring confidentiality of patient information
- Strong communication skills with the ability to observe patient conditions accurately and report findings clearly
