Independent Living Case Manager

CRAY YOUTH AND FAMILY SERVICES POSITION OPENING

Full time position. High school or GED required, but bachelor's degree preferred.

Job Responsibilities

• Provide individual and group life skills instruction
• Conduct formal and informal needs assessments
• Develop IL and transition plans
• Attend court hearings
• Ensure program compliance with all applicable laws, regulations, mandates and bulletins
• Act as a community resource link for youth
• Attend 40+ hours of training yearly
• Keep and maintain accurate client files
• Contribute as needed to NYTD data collection
• Attend the IL Retreat
• Attend local, regional and state meetings
• Valid driver's license and good driving record
• Assist youth in finding and maintaining employment
• Assist youth in securing housing
• Assist youth in educational pursuits
• Assist youth in opening bank accounts
• Assist youth in obtaining birth certificates, social security cards, photo ID's driving permits and other necessary items
• Provide services during non-traditional hours when necessary
• Report directly to the Independent Living Coordinator

Skills Required:

• Ability to work closely with legal, physical and behavioral health professionals, County Caseworkers, IL youth and community partners
• Ability to learn and apply required computer software and data entry skills
• Possess excellent interviewing and engagement techniques
• Ability to adapt to multiple demands and changing priorities and accommodate emergencies
• Ability to think and act independently or with minimal supervision
• Ability to work within the goals and mission of Cray Youth and Family Services
