Compensation:
Starting at $48,000 per year. This position also includes your housing. Impact Living Services will provide you with a Two Bedroom apartment, equalling roughly $24,000 per year.

Overview:
As the On-Site Case Worker, you become crucial in the lives of youth aged 17-21 as they navigate the intricate journey of transitioning out of foster care. In this influential position, you will be entrusted with the responsibility of ensuring the delivery of high-quality services to our youth, contributing to the overall success of the program, and upholding the utmost standards of case management.
Responsibilities:
- Provide comprehensive case management responsibilities, ensuring the provision of tailored support and guidance to individuals transitioning to independent living. This includes documenting all of the following information: coordinating resources, conducting assessments, developing individualized plans, and collaborating with relevant stakeholders.
- Supervise youth age 17-21, provide guidance and support to help them achieve their goals as identified on their service plan.
- Conduct thorough interviews and assessments with program participants and their identified family to gather essential information, understand their needs, and create individualized plans for their successful admission into the program.
- Actively fosters a collaborative and supportive atmosphere, contributing positively to team dynamics and ensuring seamless cooperation among colleagues.
- Takes initiative in creating and maintaining a positive work environment, promoting open communication, mutual respect, and a shared commitment to collective success.
- Skillfully prepare teens for both program admission and discharge, providing necessary information, support, and resources to ensure a smooth transition into and out of the program.
- Collaborate and establish partnerships with essential local entities, such as DSS, City Schools, DJJ, and medical and mental health providers, to enhance program effectiveness and support a comprehensive approach.
- Actively engage in and contribute to various tasks and responsibilities as assigned, demonstrating flexibility and a proactive approach to support the overall goals and needs of the team or organization.
Education and Experience:
- A Master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with a student placement in casework services to children and families or one year of experience in providing casework services to children and families; or
- A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families; or
- A baccalaureate degree in any field plus two years experience in providing casework services to children and families. (required)

Knowledge, Skills, and Abilities:
- Possesses a profound understanding of trauma, coupled with a high level of proficiency in applying trauma-aware practices with sensitivity and effectiveness.
- Exhibits a high level of proficiency in engaging and collaborating with youth facing social or emotional challenges.
- Exhibits a thorough understanding of the child welfare system, encompassing knowledge of Child Protective Services (CPS) and foster care.
- Possesses expertise in regulations related to licensed child placing agencies, with a specific focus on Independent Living and Treatment Foster Care.
- Demonstrates advanced proficiency in fostering positive and productive relationships, showcasing a deep understanding of effective interpersonal dynamics.
- Exhibits outstanding writing skills, articulating ideas with clarity, precision, and a keen awareness of audience engagement.

The IL case manager must meet one of the following requirements:
- A doctorate or master's degree in social work from a college or university accredited by the Council on Social Work Education or a field related to social work such as sociology, psychology, special education, or counseling, with a student placement in casework services to children and families or one year of experience in providing casework services to children and families;
- A baccalaureate degree in social work or a field related to social work including, but not limited to, sociology, psychology, special education, or counseling and one year of experience in providing casework services to children and families; or
- A baccalaureate degree in any field plus two years experience in providing casework services to children and families.
Knowledge, Skills, and Abilities:
- Must be knowledgeable of the effects of abuse/neglect on development for youth and be able to work effectively with socially or emotionally maladjusted youth.
Duties and responsibilities:
- *Note: This position requires the candidate relocate and live on site at our apartment complex. RENT for qualified candidate will also be paid for in this position.
- Interviewing youth for placement/appropriateness;
- Selecting placements in apartment or dormitory settings
- Service planning by developing service plans;
- Provide case management for youth aging out of Foster Care
- Counseling youth and families in preparation for placement or discharge;
- Supervising youth in independent living arrangements;
- Preparing and maintaining case records;
- Coordinating services to minimize fragmentation of care, reduce barriers, and link youth with appropriate services to ensure comprehensive, continuous access to needed medical, social, educational, and other services appropriate to the needs of the youth;
- Assessing periodically to determine the youth’s needs for psychosocial, nutritional, medical, and educational services;
- Linking the youth to services and supports specified in the service plan. The case worker shall directly assist the youth to locate or obtain needed services and resources. The case worker shall coordinate services and service planning with other agencies and providers involved with the child by arranging, as needed, medical, remedial, and dental services;
- Monitoring and following up by assessing ongoing progress in each case and ensuring services are delivered. The case worker shall continually evaluate and review each youth’s plan of care. The case worker shall collaborate with the family assessment and planning teams and other involved parties on reviews and coordination of services to youth and their families;
- Educating and counseling by guiding the youth and developing a supportive relationship that promotes the service plan; and
- Collaborating closely with the family assessment and planning teams and other involved parties in preparation of all case plans.
- Participating in on call and after hours assignments
- Participating in other duties as assigned.
