Independent Living Facilitator

Job Description

We are seeking an Independent Living Facilitator who will be working with new and existing consumers with all types of disabilities of all ages. Spanish speaking and / or American Sign Language experience preferred. Persons with disabilities are encouraged to apply. Position is full-time and includes company paid health and dental insurances.

EDUCATION AND REQUIREMENTS:

A college degree in Human Services, Social Services, Psychology, or Sociology is preferred. (Two plus years of qualified experience working in a field serving persons with disabilities may substitute on a year for year basis for a degree.)

Demonstrate professional and effective oral, verbal, and electronic communication skills in both English / Spanish and / or American Sign Language.

MUST have reliable transportation and a valid and current driver’s license and auto insurance.

MUST have ability to conduct home visits / attend community events.

MUST be comfortable with public speaking.

Ability to pass Level 2 Background screening.

Deadline for submission is Monday, June 1.

PRIMARY DUTIES AND RESPONSIBILITIES

· Educate consumers about Disability Achievement Center’s mission and services.

· Interact with other team members and Directors to provide comprehensive and timely assistance in resolving consumer issues, sharing information.

· Assist consumers with development of their own Independent Living Plans (ILP) with specific measurable goals and outcomes. .

· Utilize agency-approved intake, assessment, and documentation tools to identify consumer needs, barriers to independence, and appropriate supports and services.

· Provide appropriate service and advocacy support as directed by the consumer and defined by management.

· Maintain regular and active contact with consumers ensuring services sought are utilized.

· Participate in collaborative community projects on disability issues.

· Engage in promoting disability rights on the local, state and federal level.

· Attend team and staff meetings, as well as, scheduled training opportunities.

· Input documentation into database case management system.

· Assume any other responsibility as designated by the Executive Director or Director of Program Services

Schedule:

  • Monday to Friday 9AM – 5PM
  • Largo, FL 33773: Reliably commute or planning to relocate before starting work (Required)
  • This is not a virtual position. Employee expected to be in-office except when doing home visits or other out-of-office assignments.

Job Type: Full-time

Pay: $34,000.00 - $38,000.00 per year
