Position Overview:
The Initial Access Care Coordinator is responsible for supporting individuals as they seek services through Fraser. This role works closely with clients and their care teams, as well as in collaboration with mental health providers on the access team, to guide the initial assessment process and connect individuals with appropriate supportive resources.

High School diploma or equivalent required with one year of relevant work experience – health care, mental health, customer service, administrative experience.

Location: Fraser Bloomington/Virtual Office & Community-Based
Schedule: Monday-Friday, between the hours of 8 AM - 6 PM (full-time)
Pay: The starting pay range for this role is $22.60 - $23.00 an hour depending on qualifications.
