Pay: $25 – $35 per hour (DOE)

About the Role

We are looking for an experienced Home Health Intake Coordinator who can take ownership of the entire intake process—from referral to active patient—and function as a case manager to ensure patients are properly supported.

This is not an entry-level role. You must understand home health operations, move fast, and stay organized under pressure.

Responsibilities

• Manage full intake process from referral to admission
• Verify insurance eligibility (Medicare and other payors)
• Obtain physician orders/referrals or coordinate with mobile physicians
• Schedule and dispatch RN for Start of Care (SOC)
• Staff patients appropriately based on frequency and episode needs
• Review and understand diagnoses for proper care coordination
• Act as case manager:
• Coordinate with patients, families, ALFs, and care coordinators
• Arrange DME (hospital beds, supplies, etc.)
• Ensure smooth transition from intake to active care
• Maintain communication with clinical staff and ensure follow-through
• Perform charting and ensure documentation is compliant and complete
• Track patient status from Pending → Eligible → Active

Requirements

• Minimum 1 year experience in Home Health (NOT hospice)
• Hands-on experience with (required)
• Strong understanding of:
• Intake workflow
• Eligibility verification
• Physician order process
• Home health documentation & compliance
• Ability to multitask and manage multiple patients simultaneously
• Strong communication and follow-up skills
• Spanish-speaking preferred (not required)
