Community Support Services Intake/Case Manager

General Purpose

The Intake/Case Manager serves as an important first point of contact for individuals seeking assistance through Community Support Services. This position helps clients navigate programs and community resources by conducting pre-screening, scheduling appointments, assisting with applications, gathering and maintaining required documentation, monitoring caseloads, and coordinating referrals. The ideal candidate will provide welcoming, high-quality customer service; maintain accurate and confidential client records; and work respectfully with individuals from diverse socioeconomic, ethnic, and cultural backgrounds, using a trauma-informed approach. This role requires professionalism, sound judgment, strong organizational and communication skills, and proficiency with internet-based and computer applications. The position may also assist with community outreach and general administrative support as needed.

Minimum Qualifications

  • High school diploma or GED required; associate’s degree preferred
  • Case Manager Certificate or 1–3 years of experience in human services or a related field
  • Proficiency with computer applications and office procedures
  • Strong written, verbal, and interpersonal communication skills
  • Valid driver’s license and ability to travel within the Five County service area
  • Ability to pass a background check

Schedule

Full-time, in office 40 hours per week, 4–10-hour days. Monday – Thursday, 7 am to 6 pm.

Pay

Starting wage: Annual salary: $45,804
