Integrated Care Team Lead - LMHP or LADC

THE JOB
The Integrated Care Team Lead is responsible for coordinating, implementing and general oversight of the outpatient integrated health service delivery provided by an array of staff members. They will provide assessment, counseling, support services to assigned caseload, as well as in cooperation with members of the treatment team and oversee health and safety of the treatment environment. They will establish good rapport with a wide variety of agencies, organizations, and individuals. They will work closely with treatment team personnel to facilitate admission and pre-discharge planning, care coordination, and team communication. Responsible to lead, coach, and foster professional growth for team members.
