Job Summary
We are seeking a dedicated and highly skilled Internal Medicine Physician to join our dynamic healthcare team. In this vital role, you will provide comprehensive primary care services to adult patients, in assisted living facilities, and medical house calls, in Erie, and Niagara county, NY.,emphasizing prevention, diagnosis, and treatment of a wide range of medical conditions. Your expertise will help improve patient outcomes through thorough assessments, personalized care plans, and collaborative clinical practices. Responsibilities
- Conduct detailed medical history reviews and comprehensive physical examinations for adult patients across outpatient and inpatient settings.
- Develop and implement individualized treatment plans, incorporating evidence-based practices and patient preferences.
- Utilize Electronic Health Record (EHR) systems such as Epic and EMR systems to document patient encounters accurately and efficiently.
- Order and interpret diagnostic tests including medical imaging such as CT scans, radiology reports, and laboratory results to inform clinical decisions.
- Manage acute and chronic illnesses, including complex cases involving general internal medicine.
- Coordinate with multidisciplinary teams for care.
- Provide patient education on health coaching, medication management, preventive care measures, and lifestyle modifications to promote long-term wellness.
- Supervise Adult Practitioner providers.
Qualifications
- Medical degree from an accredited institution with active licensure as an Internal Medicine Physician.
- Proven experience in outpatient and inpatient internal medicine practice; hospital experience preferred.
- Proficiency with EHR systems such as Epic or similar platforms; familiarity with medical coding (CPT, ICD) is advantageous.
- Strong knowledge of physiology, medical terminology, and clinical evaluation techniques.
- Ability to handle diverse cases involving geriatrics, family medicine is desirable.
