Position Overview
The Case Manager for Substance Abuse Recovery and Mental Health Support is responsible for providing structured, evidence-based case management services to individuals in recovery from substance abuse and co-occurring mental health challenges. This role ensures that clients receive personalized support, achieve their recovery goals, and develop the skills necessary for independent living. The Case Manager will engage clients through direct interaction, resource coordination, and skill-building interventions, while maintaining accurate documentation and adhering to program standards.
Key Responsibilities:
Client Engagement and Support
  • Provide a minimum of 2 hours and up to 10 hours of direct engagement per client weekly, adjusting based on individual needs and progress.
  • Build trust and maintain positive relationships with clients to encourage active participation in their recovery journey.
  • Conduct comprehensive assessments to identify client needs, strengths, and barriers to recovery.
  • Develop and implement individualized care plans that align with client goals and program objectives.
Case Management Interventions
  • Facilitate skill-building sessions focused on independent living skills, such as time management, organizational strategies, budgeting, and job readiness.
  • Provide psychoeducation on mental health, substance abuse recovery, and relapse prevention strategies.
  • Connect clients to community resources, including housing, employment, healthcare, and peer support programs.
  • Advocate on behalf of clients to address barriers to accessing services or achieving goals.
  • Coordinate care with other service providers to ensure continuity and effectiveness of interventions.
Documentation and Compliance
  • Maintain accurate and timely documentation of all client interactions using the SOAP and DAP (Data, Assessment, Plan) framework.
  • Complete session notes within 24 hours of each interaction to ensure compliance with program standards.
  • Regularly review and update client care plans to reflect progress, new needs, or changes in circumstances.
  • Participate in monthly quality assurance reviews of documentation and care plans conducted by the program supervisor.
Follow-Up and Crisis Management
  • Schedule follow-up appointments during each session to maintain consistency and accountability.
  • Monitor client progress weekly, addressing barriers and adjusting interventions as needed.
  • Provide immediate support during crises, escalating engagement hours and collaborating with external services when necessary.
  • Document crisis incidents comprehensively and ensure follow-up measures are implemented.
Transition Planning and Aftercare
  • Develop and implement transition plans as clients achieve self-sufficiency and recovery milestones.
  • Assist clients in creating aftercare plans that include ongoing support systems, coping strategies, and relapse prevention measures.
  • Ensure clients are connected to long-term resources and support networks before program discharge.
Minimum Qualifications
  • Strong understanding of substance abuse recovery principles, mental health challenges, and evidence-based interventions.
  • Excellent organizational and time management skills to handle multiple clients and priorities effectively.
  • Ability to build rapport and maintain professional relationships with clients from diverse backgrounds.
  • Proficiency in using documentation frameworks such as DAP notes.
  • Crisis intervention skills and the ability to remain calm under pressure.
  • Knowledge of community resources and the ability to connect clients to appropriate services
Physical Requirements
  • This position requires travel to client homes, community settings, and program offices.
  • Flexible scheduling may be required to accommodate client needs, including occasional evenings or weekends
Education/Licensure Requirements
  • Bachelor’s degree in Social Work, Psychology, Counseling, or a related field (required).
  • Master’s degree in a related field (preferred).
  • Minimum of 2 years of experience in case management, substance abuse recovery, or mental health support
Starting pay: $52,000 annually
