Behavioral Health Care Manager – Collaborative Care (CoCM) Model
Location: Telehealth – Remote for 90% ~ 10% in Person
Department: Collaborative Care Management / Integrated Behavioral Health

Licensure Requirements (Must Hold One of the Following CA Unrestricted Licenses):
- Licensed Clinical Social Worker (LCSW)
- Licensed Mental Health Counselor (LMHC)
- Licensed Professional Clinical Counselor (LPCC)
- Licensed Psychologist
- Registered Nurse (RN) with Behavioral Health Experience
- Board Certified Behavior Analyst (BCBA) or BCBA-D
Job Summary
The Behavioral Health Care Manager is an essential member of the Collaborative Care (CoCM) team at ProMedix Health. This role supports the integration of behavioral health into primary care by coordinating mental health services, conducting clinical follow-ups, and collaborating with medical providers to ensure effective patient care. The position focuses heavily on psychosocial needs assessment, treatment plan monitoring, and care coordination, using a telehealth-based model to support patients remotely.
Key ResponsibilitiesPatient Assessment & Psychosocial Support
- Conduct initial and ongoing mental health assessments, emphasizing identification of psychosocial needs, barriers to care, and patient strengths.
- Utilize validated mental health screening tools and monitor patient progress throughout treatment.
Care Coordination & Treatment Planning
- Coordinate mental and physical health care with primary care providers and mental health specialists.
- Implement and update individualized treatment plans, ensuring alignment with clinical recommendations and patient needs.
- Communicate treatment adjustments to medical providers after case reviews with psychiatric consultants.
Brief Evidence-Based Interventions
- Provide short-term, structured behavioral interventions such as:
- Behavioral activation
- Problem-solving treatment
- Motivational interviewing
- Support behavioral goals, symptom reduction, and patient engagement.
Psychopharmacology Support
- Monitor adherence to prescribed psychotropic medications and track side effects or complications within provider’s scope of practice.
- Communicate patient observations to the prescribing medical provider.
Documentation & Clinical Tracking
- Use an EMR/EHR system to document encounters, interventions, treatment plan updates, and follow-up notes.
- Maintain a patient registry to track clinical progress, outreach attempts, and follow-up needs.
Referrals & External Coordination
- Facilitate referrals to psychosocial services, housing support, vocational programs, mental health specialty care, and other resources appropriate to patient needs.
Relapse Prevention & Self-Management (Separate, Not Combined)
- Relapse Prevention: Develop individualized relapse prevention strategies as patients near treatment goals.
- Self-Management: Teach patients techniques for long-term symptom management and coping.
Experience Requirements
- Minimum 2+ years of clinical experience in behavioral or mental health care.
- Experience with treatment planning, psychosocial needs assessments, behavioral interventions, and clinical follow-up.
- Experience conducting mental health assessments and screenings.
- Experience providing brief, evidence-based interventions such as motivational interviewing, behavioral activation, and problem-solving treatment.
- Basic knowledge of psychopharmacology for common mental health conditions (within appropriate scope).
- Experience with telehealth-based patient interactions and remote care delivery.
- Experience documenting in an EMR/EHR system.

Skills (Separated from Experience)
- Motivational interviewing proficiency, with ability to build rapport and support patient activation.
- Strong clinical judgment and ability to recognize psychosocial factors affecting care.
- Excellent communication skills with the ability to coordinate across multidisciplinary teams.
- Ability to maintain accurate, timely documentation in EMR/EHR systems.
- Strong organizational skills, including tracking large caseloads and managing multiple follow-ups.
- Ability to identify care gaps, advocate for patient needs, and escalate concerns appropriately.
- Skilled in developing clear, actionable relapse prevention plans and structured self-management strategies.
- High level of professionalism, cultural humility, and patient-centered care approach.
Work Environment
- Remote (Telehealth) position.
- Occasional travel may be required for trainings or program needs.
- Requires reliable internet connection and HIPAA-compliant workspace.
