Job Qualifications

Bilingual/Spanish Preferred (but not required)

· Experience in care coordination, case management, or clinical practices

· Three plus years of experience as a case manager or 1 year of similar duties

· A valid Driver's License, reliable daily vehicle, and active CA auto insurance required

· Medical terminology or background in healthcare

· Experience working with individuals with complex conditions which may include chronic physical health issues, mental illness, substance use disorders, homelessness, justice involvement and the frequent use of multiple health systems.

Job Duties

. Conduct outreach via telehealth (and in person, if needed) to build your caseload

· Interview patients by conducting Health Risk Assessments to identify their medical and social service needs

· Educate patients about their condition, medication, and give specific instructions

· Develop an individualized care plan to address their personal health & social needs

· Respond to patients' questions and expressions of concern; maintain consistent monthly contact with your assigned ECM members

· Comply with hospital and legal requirements by fostering nursing practices that adhere to the hospitals and nursing division's philosophy, goals, and standards of care; requiring adherence to nurse practice act and other governing regulations

· Work as part of a team with other members of the interdisciplinary team

· Facilitate the coordination of social services for patients and provide linkage to community resources

· Must be technology proficient- navigating software/systems independently throughout your shift and meet productivity expectations

· Perform other duties as assigned

Education

Minimum of a bachelor's degree (preferred) in an appropriate field of study

Job Type: Full-time

Pay: $22.00 - $26.00 per hour
