Job Summary:
Responsible for providing daily support to staff through trainings, meeting shadows, and being available for questions as they arise. In addition to providing contracted case management services to a partial/reduced case load in Hennepin County.
Mount Olivet Rolling Acres – MORA – is a diverse and inclusive nonprofit that has supported adults, seniors and children with developmental disabilities, behavioral health needs as well as age-related and chronic medical conditions for over 50 years in the Twin Cities metro. Due to continued growth in our Case Management department, we are seeking a Case Manager for clients in Hennepin, Anoka, & Dakota Counties. Office location is in Chanhassen.
An array of services: Adult Day Services, Contracted Case Management, Community Supports, over 30 Residential Homes, Respite, Therapeutic Recreation, Training for Guardians & Families, and a unique therapeutic Summer Camp for Youth with Autism. Our Metro Crisis Coordination Program (MCCP) offers a safety net of crucial and cost-effective services for adults and children.
Salary: $63,100 annual salary
CML Tier 2-$64,400 annual salary
CML Tier 3-$65,700 annual salary
Schedule: 40 hours, M-F daytime/business hours.

Key Duties and Responsibilities:
-
Conduct all functions as laid out in Case Manager Job Description
-
Maintains billing and audit goals for their case load
- Weekly 1:1s with identified staff (minimum of 30 minutes per staff)
-
Day to day support to staff via office hours, email, or other consultation (more in office time expected)

Provide expert support on policies, programs, and consultation to staff on team

Follow up with staff to support them with audits, billing, and other vital case management duties and goals

Conduct training for staff and shadow meetings when needed

Assures quality, person-centered services for individuals served

Participate, attend and/or chair committees, teams and or focus groups as needed or as assigned

Works harmoniously with and shows respect for employees, supervisors, providers and clients

Other duties as assigned
Minimum Qualifications:
-
Bachelor’s degree from an accredited college or university with major in Social Work, Psychology, Special Education, Nursing, Human Services, Sociology or closely related field

Qualify as a QDDP/QIDP, if working the DD waiver program
-
Waiver case management experience, specifically in a contracted setting

Proven interpersonal skills

Proven communication skills

Must understand the person-centered process

Agreement with MORA’s Mission, Vision, & Values as well as continuous quality improvement process
- Must pass Department of Human Services background check and maintain “qualified” status

Must not be excluded from working in government health programs by the Office of Inspector General

Must be able to successfully complete company’s physical job demands document

Must have a current valid driver’s license and must have insurance as required by state law
- Proven leadership skills
Desirable Qualifications:
-
Three years of experience with persons with developmental disabilities/related condition, mental health and/or aging population

Social Work licensure
-
Minimum of one-year experience as a case manager at Mount Olivet Rolling Acres

Supervisory experience
