Lead Case Manager for Los Angeles DMH Residential Treatment Program

We are seeking a Lead Case Manager to oversee the case management team at two Los Angeles County Medi-Cal DMH residential mental health treatment programs located in Sylmar and Downey. Our clients are hospital referrals, many experiencing homelessness, with diagnoses such as schizophrenia, bipolar disorder, depression, anxiety, and delusional disorders.

This role will support case managers in developing effective discharge plans, connecting clients to Los Angeles County resources, and ensuring proper documentation within our EMR system.

Responsibilities

• Supervise and support case management staff across both locations
• Assist with discharge planning and coordination of care
• Help clients access housing, benefits, medical/mental health appointments, and community resources
• Connect clients with L.A. County programs (e.g., FSP, ERS, IMD placements)
• Maintain accurate documentation in EMR systems

Requirements

• Case management experience required
• Discharge planning experience required
• Knowledge of Los Angeles County mental health and housing resources strongly preferred
• Experience with Medi-Cal or DMH populations preferred
• Mental health experience a plus
• Must live in Los Angeles County
• Must pass or have previously passed a Live Scan federal fingerprint background check
• Position is fully in-person and requires travel between both program locations (Sylmar and Downey).

Job Type: Full-time

Pay: $24.00 - $26.00 per hour
