Lead Case Manager - Spanish/English Speaking Required

Lead Case Manager - CalAIM Program

We are seeking an experienced and highly motivated person to join our team as a Lead Case Manager for the California Advancing and Innovating Medi-Cal (CalAIM) program. In this crucial role, you will provide outreach and comprehensive case management services to CalAIM recipients.

Responsibilities:

  • Conduct outreach efforts to identify and engage eligible CalAIM recipients in need of case management services
  • Develop and implement individualized care plans tailored to each recipient's unique needs and goals
  • Coordinate and facilitate access to a wide range of medical, social, and community-based services
  • Maintain accurate and up-to-date case records and documentation
  • Collaborate with interdisciplinary teams, including healthcare providers, social workers, and community partners
  • Advocate for recipients to ensure they receive the necessary support and resources
  • Provide education and guidance to recipients and their families on available services and programs
  • Participate in case conferences and team meetings to discuss and review case progress

Qualifications:

  • Current Licensed Vocational Nurse (LVN) license in good standing HIGHOY PREFERRED
  • Ability to speak Spanish and English, required
  • Experience in case management appreciated, however we are able to train
  • Excellent communication and interpersonal skills, with the ability to build rapport with diverse populations
  • Proficiency in computer applications and electronic medical record systems
  • Demonstrated ability to work independently and as part of a team
  • Organized, detail-oriented, and able to manage multiple tasks and priorities
  • Compassionate and patient-centered approach to care
  • Willing to travel across the east and west areas of Los Angeles County
