Lead RN Case Manager (Bilingual)

This position is for a professional who will be responsible for expertly coordinating all aspects of patient care and facilitating our Interdisciplinary Team (IDT) meetings. Primary Responsibilities: Care Coordination & ManagementPatient Care Coordination & IDT Facilitation

  • Serve as the central clinical leader and care coordinator for a defined patient caseload, managing the entire care episode from admission through discharge.
  • Lead and facilitate the Interdisciplinary Team (IDT) meetings, ensuring comprehensive patient review, updating the Plan of Care (POC), and documenting team consensus on goals and service needs.
  • Proactively coordinate the interdisciplinary team (field nurses, therapists, social workers) to ensure synchronized care delivery, timely scheduling, and smooth patient transitions.
  • Facilitate effective communication between patients, families, physicians, payers, and field staff to promptly address needs and resolve service gaps.
  • Process and manage physician orders and changes to the Plan of Care efficiently and accurately.

Quality Monitoring & Compliance

  • Perform as needed concurrent monitoring and compliance checks (Active Case Review) of clinical documentation to ensure the POC is clinically appropriate and executed correctly by the team.
  • Review and approve OASIS assessments and 485s, verifying medical necessity and compliance before submission.
  • Act as a clinical resource and support for field nurses, providing guidance on complex cases to ensure high-quality care delivery.
  • Perform utilization review activities to manage the length of stay and ensure the appropriate level of necessary care is delivered.

On-Call Duty (Mandatory)

  • Participate in a rotating on-call schedule (evenings, weekends, holidays) to provide essential clinical consultation and support for staff and patients after hours.

Qualifications: Your Experience is Our Future Required Experience & Skills

  • Current, unrestricted Registered Nurse (RN) license in California.
  • Bilingual (Spanish and English)
  • Minimum of 3 years of recent RN experience in total.
  • Minimum of 2 years of experience specifically in Home Health.
  • Excellent customer service experience demonstrated through effective, compassionate communication with patients, families, and physicians.
  • Strong experience in Home Health care coordination, case management, and IDT facilitation.
  • Proven ability to manage a full caseload and meet productivity expectations in an office setting.
  • Proficiency in using Electronic Health Records (EHR) systems.
  • Must be willing and available to participate in the mandatory rotating on-call schedule.

Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Strong working knowledge of Joint Commission and Medicare guidelines/standards related to quality patient care and agency operations.
  • Prior experience in a smaller, independent home health agency.
  • In-depth knowledge of Medicare/Medi-Cal regulations, OASIS guidelines, and reimbursement rules.
  • Local knowledge of community resources in the Reseda/San Fernando Valley area.

Pay: $45.00 - $47.00 per hour
