Licensed Bachelor Social Worker - Behavioral Health Case Manager

Seeking Licensed Social Worker to join our Assertive Community Treatment team in Paw Paw to provide case management services to dually diagnosed adults. Candidates must have an appropriate Bachelor's Degree with LLBSW or LBSW Michigan licensure and at least one year of experience working with individuals with serious mental illness and/or substance use disorder.

The Case Manager Advocate coordinates community treatment and therapeutic interventions, including person-centered and recovery-oriented care, stage-wise interventions and motivational interviewing techniques. Will link & coordinate with community agencies to assist customers in obtaining benefits & housing resources. Coordinates/leads ACT/IDDT treatment groups. Evaluates customer needs for leisure time skills, community support and health care and acts as a liaison to ensure customer's needs for independent living and purposeful activity are met. Frequent travel throughout the county required. The Case Manager Advocate will also provide after-hours on-call on a rotating basis with additional compensation.

Position is full-time (37.5 hours per week).
