We are seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our behavioral health team in Dade City, Florida. This position provides an opportunity to make a meaningful impact by helping individuals and families navigate behavioral health challenges while connecting them with essential community resources and support services.

The LCSW will provide counseling, diagnostic evaluations, case management services, and crisis intervention support for children, adolescents, and adults. This role works collaboratively with a multidisciplinary healthcare team to promote mental wellness and improve overall patient outcomes.

Position Summary

Licensed Clinical Social Workers provide behavioral health counseling and support to patients experiencing mental illness, addiction, abuse, family instability, chronic stressors, and other life challenges. This role focuses on diagnosing, treating, and managing acute and chronic behavioral health conditions while promoting overall community wellness.

Essential Duties and Responsibilities

• Maintain a productive daily patient schedule
• Conduct diagnostic evaluations and behavioral health assessments
• Review medical histories and treatment plans
• Provide individual counseling and therapy services
• Provide case management and resource coordination services
• Communicate with healthcare providers, schools, agencies, and community partners as needed
• Participate in crisis intervention and care coordination activities
• Document patient encounters accurately within the Electronic Health Record (EHR) within 48 hours
• Review and address daily tasks and documentation requirements
• Enter billing charges in a timely manner
• Educate patients and families regarding community resources and available support services
• Collaborate with Behavioral Health leadership and fellow clinicians to ensure quality patient care
• Maintain patient confidentiality and HIPAA compliance
• Support community outreach initiatives and health education programs
• Participate in quality improvement and quality assurance activities
• Perform other duties as assigned

Qualifications

Required

• Master of Social Work (MSW) degree from an accredited program
• Active Florida Licensed Clinical Social Worker (LCSW) License
• Current Basic Life Support (BLS) Certification
• Knowledge of behavioral health counseling principles and practices
• Ability to work effectively with diverse patient populations

Preferred

• Two (2) years of counseling and resource coordination experience in a community healthcare setting
• Experience working with children, adolescents, and adults
• Experience with Electronic Health Record (EHR) systems
• Experience in integrated behavioral health or primary care settings
• Ability to maintain required compliance and training certifications

Working Conditions & Physical Requirements

• Ability to sit for extended periods
• Frequent use of computers and digital screens
• Ability to lift up to 20 pounds regularly and up to 30–50 pounds occasionally
• Potential exposure to contagious and infectious diseases in a healthcare setting
• Ability to travel between locations as needed

Background Screening Requirement

This position requires successful completion of a Level 2 Background Screening through the Florida Agency for Health Care Administration (AHCA) Care Provider Background Screening Clearinghouse.

Applicants may be required to meet all eligibility standards established by AHCA and applicable state regulations.
