Licensed Clinical Social Worker (LCSW) Consultant

Miracle Adult Day Health Care (ADHC) is seeking a Licensed Clinical Social Worker (LCSW)- Consultant to join our team in San Bernardino, CA! We provide services to help individuals with developmental disabilities develop their social and emotional skills and are a valued part of the community. We are looking for someone who wants to make a difference in the lives of our clients and their family members.

Purpose

In order to provide appropriate treatment for participants diagnosed with mental, emotional or behavioral problems, consultant psychiatric and psychological services will be provided.

Responsibilities include but not limited to:

• Psychiatric assessment and reassessment of participants when indicated.
• Develop a person-centered treatment plan taking into consideration the health needs including problems and goals.
• Implementation of treatment plan.
• Consultation with other members of the team, family members, and referral agencies. Ongoing training of center staff. Provide at least 3 hours per month of staff consultation and trainings.
• Maintenance of signed "Progress Notes" in participants'' charts on at least a quarterly basis when appropriate. Treatment shall be documented on the same day services are given.
• May provide direct service to the participant as indicated.

Qualifications

• LCSW services are provided by a registered nurse licensed to practice in California.
• Liability and malpractice insurance as appropriate in the minimum amounts of $200,000/$600,000

LCSW is available during all hours of operation.
