Job Summary

Millennium Home Health is seeking a dedicated and compassionate Licensed Physical Therapist (PT) or Occupational Therapist (OT) to join our growing home health team serving Ventura County and surrounding areas. This role offers the opportunity to provide high-quality, patient-centered therapy services directly in patients’ homes, supporting recovery, independence, and overall quality of life.

You will work with children, adults, and geriatric patients, including individuals with developmental disabilities, delivering individualized care in a supportive and flexible home health environment.

Responsibilities

• Conduct comprehensive patient evaluations and functional assessments
• Develop and implement individualized treatment plans in accordance with physician orders and evidence-based practice
• Provide skilled PT or OT services in the home setting
• Educate patients and caregivers on exercises, safety techniques, and home programs
• Collaborate with physicians, case managers, and interdisciplinary care teams
• Maintain accurate, timely documentation in compliance with HIPAA and home health standards
• Participate in care coordination, reassessments, and discharge planning
• Provide in-home services and telehealth visits when appropriate

Qualifications

• Valid California license as a Physical Therapist (PT) or Occupational Therapist (OT)
• Experience working with children, adults, geriatrics, or individuals with developmental disabilities
• Home health experience preferred but not required
• Strong clinical, documentation, and communication skills
• Ability to work independently in a home-based setting
• Reliable transportation and willingness to travel within Ventura County

• $145,000 – $160,000 annually (full-time equivalent)
