RoseVilla is a Residential Care Facility that provides Emergency Housing and Long Term Care to veterans. This position is open for LMSW in Kansas. Recent graduates who have taken their exam and are only waiting on their official transcripts are also encouraged to apply.

Job Description

The social worker will work in conjunction with the veteran and VA support team to develop a highly Individualized Service (ISP) . The social worker will work with the VA Liaison, Program Director and Veteran with the goal of finding permanent sustainable housing with in 60 days.

Provide case management for homeless veterans to include:

Conduct weekly meetings with each resident to review ISP and progress. This can include helping residents get identity documents, applying for VA or Social Security benefits, employment, or medical/mental health appointments. Promote a safe, healthy, positive, goal-oriented living environment
Meet with VA Liaison once weekly to communicate resident progress towards goals.
Attend weekly HCHV Treatment Team meeting
Co-facilitate a weekly "town hall" type meetings with the residents.
Attend once monthly virtual team meeting.
Attend once monthly Human Services Council meeting.
Communicate with the Program Director regarding policies, HR and resident, staff issues.
Conduct new resident assessments, create file and maintain documentation
Utilize Conflict Resolution when needed
Crisis Intervention
Coordinate and assist in transportation requests (VA and non-VA)
Coordinate discharges, arrange for moving assistance, transportation, assist with locating furniture and supplies.

Training oversite and support will be provided by Rosevilla management.

Job Type: Full-time

Pay: $27.55 - $33.18 per hour
