Licensed Medical Social Worker - Contract

Job Summary
We are seeking a dedicated and compassionate Licensed Medical Social Worker to join our team on a contract basis. Responsibilities

• Conduct thorough patient assessments, including diagnostic evaluations and mental health screenings, to identify psychosocial needs and develop tailored care plans.
• Provide individual, group, and family counseling sessions utilizing evidence-based techniques such as cognitive behavioral therapy (CBT), motivational interviewing, and behavioral therapy.
• Assist patients with crisis intervention, managing acute emotional or behavioral issues with sensitivity and professionalism.
• Coordinate with interdisciplinary teams to facilitate seamless care transitions, including discharge planning and post-acute care arrangements.
• Maintain accurate medical documentation in compliance with HIPAA regulations, including detailed case notes, progress reports, and treatment plans within electronic medical record (EMR) systems.
• Collect data for clinical research projects or quality improvement initiatives related to behavioral health or hospice care.
• Support patients dealing with complex conditions such as dementia, Alzheimer's disease, PTSD, autism spectrum disorder, or disabilities by providing specialized social work interventions.
• Engage in community outreach and home visits when appropriate to deliver home & community care services that promote patient well-being outside of clinical settings.

Requirements

• Valid license as a Medical Social Worker (LMSW or LCSW) issued by the appropriate licensing board.
• Proven experience in hospital settings, hospice care, inpatient units, nursing homes, or outpatient clinics.
• Strong knowledge of medical terminology, ICD-10/ICD-9 coding systems, CPT coding for documentation purposes, and familiarity with HIPAA regulations.
• Experience working with diverse populations including children, geriatrics, individuals with developmental disabilities or autism spectrum disorder, and those requiring Alzheimer’s or dementia care.
• Proficiency in EMR systems for data collection and medical record management.
• Excellent communication skills for conducting intake assessments, psychotherapy sessions, crisis management, and case management activities.
• Ability to handle sensitive situations with empathy while maintaining professional boundaries during crisis intervention or behavioral health crises.
• Knowledge of behavioral health modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), motivational interviewing, and group therapy techniques.
• Experience working within special education programs or outpatient clinics is preferred.
• Strong organizational skills to manage multiple cases efficiently while adhering to documentation standards and confidentiality requirements.
