Wellness Homes of Chicago is seeking a Nurse Practitioner (APRN) or Physician Associate (PA) with a passion for obesity, weight management, and cardiometabolic preventive care. We are looking for an APRN or PA with 1+ years of experience and a desire to learn and a passion for making healthcare better. You should have a strong clinical and non-clinical judgment, be thorough, and take the time to address the specific needs of each individual patient.

1 FT Provider: Assigned to Back of the Yards (BOY).
1 FT Float: Covering Halsted, BOY, and weekend clinics.
This role requires someone who is deeply empathetic and humanistic, and enjoys working in a fast-paced, high-energy organization. In this role, you will provide world-class care to patients with obesity, hypertension, hyperlipidemia, diabetes, and coronary artery disease. You will also collaborate with the Care Team, including physicians, medical assistants, clinical guides, pharmacists, and pharmacy techs, to deliver holistic, patient-centered care.
Assessment: Conducting focused assessments of patients with obesity conditions that impact cardiometabolic health.
Treatment Planning: Formulating accurate diagnoses and developing individualized treatment plans for patients with obesity and cardiometabolic conditions.
Diagnostics: Ordering diagnostics, interpreting labs and imaging data, and developing treatment plans in collaboration with the specialty care clinic team.
Weight Loss Protocols: Implementing Dr. MO's 20N8 Weight Loss Plan while improving Cardiometabolic markers.
Chronic Care Management: Helping patients manage diabetes and providing diabetes education.
Risk Mitigation: Mitigating the risk of obesity and cardiometabolic conditions by proactively managing medication adherence and identifying possible contraindications.
Research & Support: Assisting with the management of the Center's Obesity and Cardiometabolic Clinical Research endeavors.
Clinical Advice: Serving as the clinical advisor and providing clinical escalation support for specialty clinic staff and other teams during business hours.
Education: Master's or Doctoral degree in Nursing or Physician Associate specializing in obesity, endocrinology, primary care, or cardiovascular care.
Licensure: An active and clear Illinois RN, APRN, or PA license.
Certification: Active BLS is required at the time of hire and must be maintained while employed.
Experience: 1+ years of experience is required. Experience in endocrinology or a subspecialty with a heavy focus on obesity, hypertension, and lipid management is highly valued.
Technology: Proficiency in using telehealth technology and EMR systems (eClinicalWorks preferred).
