Job Summary
We are seeking a dedicated and energetic Physical Therapist to join our dynamic healthcare team. Responsibilities
- Conduct thorough patient assessments to develop tailored care plans that address individual needs across various settings such as hospitals, clinics, home health, and outpatient facilities
- Implement physiotherapy interventions including therapeutic exercises, trigger point therapy, manual therapy techniques, and mobility training
- Collaborate with multidisciplinary teams to coordinate discharge planning and ensure seamless transitions between care environments
- Utilize electronic medical records (EMR) systems to document patient progress, treatment plans, and clinical notes accurately
- Provide specialized care for diverse populations including pediatrics, geriatrics, orthopedics, and patients recovering from acute care or surgery
- Educate patients and their families on injury prevention, physiotherapy exercises, and home care strategies to promote long-term wellness
- Stay current with medical terminology updates and advancements in physiology knowledge to deliver evidence-based treatments
Experience
- Proven experience in inpatient hospital settings with a strong understanding of acute care procedures and protocols
- Demonstrated success in outpatient clinics or home & community care environments delivering patient-centered physiotherapy services
- Familiarity with EMR systems for efficient documentation and treatment planning
- Hands-on experience with discharge planning processes for diverse patient populations
- Knowledge of specialized areas such as orthopedics, geriatrics, pediatrics, and trigger point therapy is highly desirable
- Strong background in anatomy knowledge and physiology fundamentals underpinning effective physiotherapy practices
- Prior work experience in home health or hospital environments that emphasizes patient care excellence
