Licensed Practical Nurse

Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) for Day Shift to join our healthcare team. The ideal candidate will provide high-quality patient care in various settings, including hospitals, nursing homes, assisted living facilities, and home care environments. The LPN will work collaboratively with healthcare professionals to ensure optimal patient outcomes while adhering to best practices and regulatory standards.

Responsibilities

• Administer medications and treatments as prescribed by physicians.
• Perform patient assessments and monitor vital signs.
• Assist in the development and implementation of patient care plans.
• Maintain accurate medical documentation and records in compliance with HIPAA regulations.
• Collaborate with interdisciplinary teams to coordinate patient care.
• Educate patients and families on health management and disease prevention.
• Ensure a safe and clean environment for patients by following infection control protocols.

Skills
Candidates should possess a combination of required and preferred skills to excel in this role, including but not limited to:

• Proficiency in EMR/EHR system PCC.
• Experience in critical care, long-term care, assisted living, hospice care, or home health settings.
• Knowledge of medication administration.
• Experience working with geriatrics, Alzheimer's care, and dementia care.
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
