Responsibilities
- Deliver comprehensive patient care by performing physical examinations, vital signs monitoring, and health assessments with precision and compassion.
- Administer medications and injections safely while adhering to established protocols and HIPAA regulations to protect patient confidentiality.
- Manage medical documentation accurately using EMR (Electronic Medical Records) systems like Epic, Cerner, or Athenahealth to ensure seamless communication across healthcare providers.
- Educate patients and their families on health coaching topics including medication management, nutrition, dementia care (including Alzheimer’s and memory care), and discharge planning for outpatient or post-acute settings.
- Participate in infection control practices and aseptic techniques during all clinical procedures to promote a safe environment for patients and staff.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification to practice.
- Proven experience in diverse clinical settings such as hospitals (including ICU or emergency room), nursing homes, long-term care facilities, or outpatient clinics.
- Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks for documentation and patient records management.
- Strong knowledge of physiology, anatomy, pharmacology related to medication administration, tube feeding techniques, and vital signs assessment.
- Experience with specialized care areas including Alzheimer's/dementia care, hospice services, pain management protocols, infectious disease control, pediatric or neonatal care is highly desirable.
- Ability to perform technical skills such as IV insertion (IV infusion), phlebotomy (venipuncture), catheter care, suturing, and sterile technique adherence.
- Excellent communication skills for triage assessments, patient education, family interactions, and interdisciplinary collaboration.
- Working knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures performed in outpatient or surgical settings.
- Physical ability to stand for extended periods; capable of lifting or repositioning patients using assistive devices like Hoyer lifts when necessary.
