We are currently seeking a Licensed Practical Nurse (LPN) to join our team. This role is ideal for someone who thrives in a collaborative environment and is passionate about supporting patients and providers through high-quality, compassionate care.

Summary

The Licensed Practical Nurse (LPN) is responsible for delivering direct patient care and supporting the daily clinical operations of JPCHC. The LPN assists providers by performing assessments, administering medications, coordinating lab work, maintaining documentation, and ensuring smooth patient flow. This position also contributes to JPCHC’s mission to deliver high-quality, patient-centered care in accordance with clinical standards, safety protocols, and regulatory requirements.

Essential Duties and Responsibilities

  • Perform patient assessments, obtain vital signs, and complete documentation per scope of practice.
  • Support chronic disease management and population health initiatives through patient education and outreach.
  • Administer immunizations, medications, and treatments in accordance with provider orders and JPCHC protocols.
  • Provide patient education regarding medications, chronic condition management, preventive care, and discharge instructions.
  • Assist with patient flow management, collaborating with providers and care teams to optimize efficiency.
  • Accurately document patient care and communications in the electronic health record (EHR).
  • Coordinate follow-up related to labs, imaging, referrals, and care transitions.
  • Adhere to infection control, safety, and HIPAA compliance standards.
  • Conduct routine screenings and point-of-care testing (e.g., blood glucose, hemoglobin, urine tests).
  • Participate in quality improvement activities, audits, and team-based care initiatives.
  • Contribute to care team huddles and daily planning meetings to ensure comprehensive patient care.
  • Precept and mentor new clinical support staff to promote teamwork and clinical excellence.
  • Perform other duties as assigned by the Practice Manager or Clinical Supervisor.

Education and Experience

  • High school diploma or equivalent required.
  • Current state licensure as a Licensed Practical Nurse (LPN) required.
  • 1–2 years of clinical nursing experience required; experience in primary care, pediatrics, women’s health, behavioral health, or community health strongly preferred.
  • Current BLS certification required and must be maintained during employment.

Job Type: Full-time

Pay: From $24.00 per hour

Expected hours: 40 per week
