Licensed Practical Nurse

Overview
Greater Meridian Health Clinic, Inc. seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with experience across various clinical settings, including long-term care, acute care, and community health. As an LPN, you will play a vital role in delivering high-quality nursing services, supporting patient recovery and well-being through comprehensive assessments, medication administration, and personalized care plans. Responsibilities

  • Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform individualized care plans.
  • Administer medications and injections accurately, including IV infusions and catheterizations, adhering to established protocols and safety standards.
  • Utilize Electronic Health Record (EHR) systems such as Epic, Cerner, or Athenahealth to document patient information meticulously and ensure compliance with HIPAA regulations.
  • Assist with diagnostic evaluations, specimen collection, and processing while maintaining strict infection control practices.
  • Collaborate with multidisciplinary teams to coordinate discharge planning, case management, and community-based home & community care services.
  • Educate patients and their families on health coaching topics such as nutrition, medication management, and chronic disease prevention.
  • Maintain accurate medical documentation of patient care activities while ensuring compliance with all regulatory standards.

Experience

  • Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
  • Demonstrated experience working in diverse clinical environments such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or outpatient clinics.
  • Proven proficiency with EMR/EHR systems like Epic, Cerner, or eClinicalWorks for documentation and patient records management.
  • Critical care experience including ICU or emergency room settings is highly desirable.
  • Experience in specialized areas such as infectious disease care, pain management, pediatrics (including neonatal ventilator support), or behavioral health is preferred.
  • Familiarity with ICD coding (ICD-9/10), CPT coding for procedures like suturing or spinal taps is advantageous.
  • Knowledge of sterile techniques such as phlebotomy, specimen collection/processing, and aseptic procedures is essential for ensuring safe patient interventions.
