Job Overview
We are looking for a nurse to provide care for a medically fragile patient at home. The schedule would be 12-hour shifts 5 to 7 days a week and also open to 16-hour shifts if staff desires, accompanying the patient at home and ensuring their clinical needs are safely managed throughout the day.
This position would be structured as a 1099 (independent contractor).

Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately, including IV infusions and botulinum toxin treatments, following strict protocols and HIPAA regulations.
- Assist with medical procedures such as catheterization, suturing, tube feeding, spinal taps, and ventilator management in accordance with aseptic techniques.
- Manage patient documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Cerner, Athenahealth, or eClinicalWorks to ensure accurate medical records and coding (ICD-9/10, CPT).
- Conduct patient care activities in various environments including acute care units, ICU (Intensive Care Unit), emergency rooms, hospice settings, and outpatient clinics.
- Support triage processes by evaluating patient symptoms for urgent or emergent needs across emergency medicine and urgent care settings.
- Collaborate with multidisciplinary teams to develop discharge planning, case management strategies, and post-acute care plans for patients with complex needs such as dementia or infectious diseases.
Experience
- Valid Licensed Practical Nurse (LPN) license with current state licensure.
- Prior experience working in diverse healthcare environments such as hospitals (including Level I Trauma Centers), nursing homes, long-term care facilities, or assisted living communities.
- Demonstrated proficiency in medical-surgical nursing, geriatrics, pediatrics (including neonatal ventilator management), behavioral health, or home & community care.
- Experience with EMR/EHR systems such as Cerner, Athenahealth, or eClinicalWorks for accurate medical documentation and coding practices.
- Knowledge of physiology and anatomy fundamentals to support patient assessments and diagnostic evaluations.
- Skilled in medication administration techniques including injections, venipuncture, IV insertion/infusion, catheter care, and sterile processing procedures.
- Familiarity with specialized areas such as Alzheimer’s/dementia/memory care, pain management—including acute pain—and infection control protocols.
- Ability to work effectively with individuals with disabilities or developmental challenges across various settings including assisted living or home care environments. Pay: $30.00 - $33.00 per hour
