The Licensed Practical Nurse for our Mobile Medical Community Health & Wellness program will provide street medicine to individuals experiencing homelessness in Oklahoma City/Tulsa as well as those residing in the Association’s permanent supportive housing units. The Licensed Practical Nurse will deliver a point-of-contact, integrated healthcare model for vulnerable individuals. In collaboration with community partners providing telehealth (e.g., Homeless Alliance and The University of Oklahoma), the Licensed Practical Nurse will implement a community-wide diversion strategy to provide episodic care for some of Oklahoma City/Tulsa’s most vulnerable and uninsured community members.
PRINCIPAL DUTIES AND RESPONSIBILITIES:

The role of the LPN includes the following:
- Making face-to-face contact with program participants as needed, meeting individuals where they are whether they are living on the street or in supportive housing apartment units, with systematic medication management services related to identified behavioral health and substance abuse concerns.

Assisting program participants with visits to physicians or other medication prescribers as needed.
- Assisting with care coordination of program participants within the context of an interdisciplinary treatment team.

Assisting participants as needed with health care assessments, medical forms and/or consents, education about health issues, and communication with health care, social services, or mental health providers.
- Consulting and assisting our Street Outreach & Rapid Response team, Service Coordinators, and Case Managers as needed with crisis intervention and mental health emergencies.
KNOWLEDGE, SKILLS & ABILITIES:

Education:
- High School Diploma or GED required.

Associate’s degree in nursing, social work, psychology, or a related field.

Experience:
-
Experience with program housing and/or working with persons with mental illness preferred.

Skills:
-
Demonstrated knowledge and skill in working with individuals with mental illness, substance abuse, and co-occurring disorders.

Thorough knowledge of professional nursing practice techniques, medical supplies, sanitation and personal hygiene, and nutrition.
- Demonstrated skill in planning, organizing, and applying nursing care techniques working with clients with mental illness and co-occurring disorders.
- Excellent oral and written communication skills.
- Demonstrated skill in exercising judgment, initiative self-control, patience in following detailed or written instructions.

Demonstrated skill in maintaining a sympathetic and harmonious relationship with individuals who are homeless and may have serious mental illnesses and co-occurring disorders.
- Certification:
- Current authorization in the State of Oklahoma as a Licensed Practical Nurse or Registered Nurse.
WORK CONDITIONS & PHYSICAL DEMANDS:

Work is performed in climate-controlled settings, where exposure to conditions of extreme heat/cold, poor ventilation, fumes and gases is unlikely.
- Noise level is moderate and includes sounds of aggregate residential living, including multiple conversations, ringing phones, intercom announcements.

Staff will report any environmental hazards encountered in normal performance of job duties and will not handle or otherwise be exposed to hazardous conditions.

Work requires a flexible work schedule.
- Ability to effectively communicate orally.

Physical ability to move 25 pounds.
